Brian Clemens Memorial Golf Tournament Online Registration

Welcome! Please fill out and submit the following form to register for the tournament. Note that your registration is not official until we have received your payment of $68 per player. Please follow the instructions below.


For everyone:

We must have some way to contact you! If you have an e-mail address, your phone number is not required. If you do not have an e-mail address to use, type N/A for your e-mail address and enter your phone number

Please enter each person's approximate average score (not the handicap!) in the appropriate spot. (If someone has never played before or does not know an approximate average score, enter "130").

If there are any special circumstances or you have any comments, questions, or concerns about registration, place them in the comments box. Then click register to submit your information!

If your are registering by yourself:

Just fill in your information in the spot for "Player 1." If you are willing to be a captain if necessary, check the "Captain" box.

If you are registering part of a team:

If you are registering two or three players who wish to be on the same team, simply fill in the appropriate information for each player. At least one person on the team must have valid contact information. If any of you are willing to be a captain, then place them in the spot for "Player 1" and check the "Captain" box. Otherwise, just place yourself in the "Player 1" spot and the other players in other spots. If there is any person that we need not contact (i.e., a person which a teammate will inform of any updates, billing information, and any other important information) simply put N/A in the spot for his or her e-mail address. If any player does not have an e-mail address and he or she wants to be contacted, please give his or her phone number.

If you are registering an entire team:

If you are registering an entire team, simply fill in the appropriate information for each player. First, you may choose a team name for your team. Then, you must pick a captain. The captain must have valid contact information, as he or she will be in charge of communications with team members. Place that player in the "Player 1" slot and check the "Captain" box. If there is any person that we should not contact (i.e., a person which a teammate will inform of any updates, billing information, and any other important information) simply put N/A in the spot for his or her e-mail address. If any player does not have an e-mail address and he or she wants to be contacted, please give his or her phone number.

If you are registering other players on your team but do not yet have their names:

If you are registering other players on your team but do know have their names, please enter your name in the Player 1 slot including either your email or phone number. We must have some way to contact you! For each additional player that you wish to register, type in your last name followed by the number that player would be on your team (numerals "2", "3", or "4").
